Ilevro Ophthalmic Suspension contains Nepafenac 0.3% w/v, a nonsteroidal anti-inflammatory drug (NSAID) indicated for the treatment of ocular inflammation and pain associated with cataract surgery. Nepafenac is a prodrug that is converted by ocular tissue hydrolases to amfenac, an active metabolite that inhibits cyclooxygenase enzymes COX-1 and COX-2, thereby reducing prostaglandin synthesis which mediates inflammation and pain. Ilevro is administered as a topical ophthalmic suspension, typically dosed once daily beginning one day prior to cataract surgery, continued on the day of surgery, and for up to two weeks postoperatively. This dosing regimen facilitates effective control of postoperative inflammation and reduces the risk of ocular pain. The formulation’s 0.3% concentration allows for enhanced penetration and sustained therapeutic levels within ocular tissues. Ilevro is contraindicated in patients with hypersensitivity to Nepafenac or any component of the formulation. Common adverse reactions include transient burning or stinging upon instillation, conjunctival hyperemia, and blurred vision. Caution is advised in patients with compromised corneal integrity or those using other topical ophthalmic medications.
