Chloramphenicol acts by inhibiting bacterial protein synthesis through binding to the 50S ribosomal subunit, thereby preventing peptide bond formation and bacterial growth. It is indicated for the treatment of serious infections such as typhoid fever, meningitis, rickettsial infections, and certain respiratory tract infections where other antibiotics are ineffective or contraindicated.

Due to its potent antibacterial activity, Kemnicol 500 should be used under strict medical supervision, especially in patients with pre-existing hematological disorders, as it carries a risk of bone marrow suppression, including aplastic anemia. Dosage must be carefully adjusted based on the severity of infection, patient age, and renal and hepatic function. Common side effects include gastrointestinal disturbances, hypersensitivity reactions, and reversible bone marrow suppression.
Periodic blood counts are recommended during prolonged therapy. Chloramphenicol is contraindicated in neonates and during pregnancy unless absolutely necessary, due to the risk of gray baby syndrome. Patients should complete the full prescribed course to prevent resistance development.
