It acts by selectively inhibiting inosine monophosphate dehydrogenase, a key enzyme in the de novo synthesis of guanine nucleotides, thereby suppressing the proliferation of T and B lymphocytes. This immunomodulatory effect reduces the risk of acute and chronic rejection in patients undergoing renal, cardiac, or hepatic transplantation.

Immudapt 500 is typically administered orally in combination with other immunosuppressants such as cyclosporine and corticosteroids to enhance efficacy. The dosage and duration of therapy are individualized based on the type of transplant, patient response, and tolerability. Common adverse effects include gastrointestinal disturbances (nausea, diarrhea, vomiting), hematological abnormalities (leukopenia, anemia), and increased susceptibility to infections.
Regular monitoring of complete blood counts and renal function is recommended during treatment. Caution is advised in patients with active infections or hypersensitivity to mycophenolate mofetil. Due to its teratogenic potential, Immudapt 500 is contraindicated during pregnancy and in women of childbearing potential not using effective contraception.
