Metformin, a biguanide, primarily acts by decreasing hepatic glucose production and improving peripheral insulin sensitivity, thereby enhancing glucose uptake and utilization. Pioglitazone, a thiazolidinedione, functions as a peroxisome proliferator-activated receptor gamma (PPAR-γ) agonist, improving insulin sensitivity in adipose tissue, muscle, and the liver.
