Metformin reduces hepatic glucose production and enhances peripheral insulin sensitivity, lowering blood glucose levels.
Sitagliptin inhibits DPP-4, increasing incretin levels, which stimulate insulin release and reduce glucagon secretion.
Medical applications
Xigtin 50mg/500mg Tablet is a combination oral hypoglycemic agent containing Sitagliptin 50mg and Metformin 500mg, indicated for the management of type 2 diabetes mellitus. Sitagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that enhances the levels of incretin hormones, thereby increasing insulin release and decreasing glucagon secretion in a glucose-dependent manner. Metformin, a biguanide, primarily acts by decreasing hepatic glucose production, improving insulin sensitivity, and enhancing peripheral glucose uptake. The combined action of these agents helps in achieving better glycemic control compared to monotherapy. Xigtin is prescribed as an adjunct to diet and exercise to improve glycemic parameters in patients inadequately controlled on either agent alone or when combination therapy is appropriate. It is important to monitor renal function prior to and during treatment due to the risk of lactic acidosis associated with Metformin. Common adverse effects may include gastrointestinal disturbances such as nausea, vomiting, diarrhea, and abdominal discomfort. Patients should be advised to report any signs of hypoglycemia, although the risk is low with this combination. Xigtin should be used cautiously in patients with renal impairment, hepatic dysfunction, or cardiovascular disease. Regular monitoring of blood glucose and HbA1c levels is recommended to assess therapeutic efficacy.
