Metformin, a biguanide, primarily decreases hepatic glucose production and enhances peripheral glucose uptake and utilization, contributing to improved insulin sensitivity. Pioglitazone, a thiazolidinedione, acts as an insulin sensitizer by activating peroxisome proliferator-activated receptor gamma (PPAR-γ), which modulates the transcription of insulin-responsive genes involved in glucose and lipid metabolism.

The combined action of these agents addresses multiple pathophysiological defects in type 2 diabetes, including insulin resistance and impaired insulin secretion. Owiglit-PG2 is prescribed as an adjunct to diet and exercise to improve glycemic control in patients inadequately controlled on monotherapy or dual therapy. Caution is advised in patients with hepatic, renal, or cardiac impairment, and regular monitoring of blood glucose, liver function, and hematologic parameters is recommended.
