By selectively blocking endothelin receptors ETA and ETB, Macitentan reduces vasoconstriction and inhibits vascular remodeling, thereby decreasing pulmonary arterial pressure and improving exercise capacity. This medication is indicated for patients diagnosed with WHO Group 1 PAH to delay disease progression and reduce the risk of hospitalization.

Macikad is administered orally, typically once daily, with or without food, and dosage adjustments should be guided by clinical response and tolerability. Common adverse effects may include headache, nasal congestion, anemia, and elevated liver enzymes. Due to its teratogenic potential, Macikad is contraindicated in pregnancy, and effective contraception is mandatory for women of childbearing potential during treatment and for a specified period after discontinuation.
Regular monitoring of liver function tests and hemoglobin levels is recommended to detect potential hepatotoxicity and anemia early. Macikad should be used cautiously in patients with hepatic impairment and is not recommended in severe liver dysfunction.
