Pioglitazone acts by improving insulin sensitivity in peripheral tissues, primarily muscle and adipose tissue, thereby enhancing glucose uptake and utilization. It functions as a selective agonist for peroxisome proliferator-activated receptor gamma (PPAR-γ), modulating the transcription of insulin-responsive genes involved in glucose and lipid metabolism.

K-Pio 30 is prescribed as monotherapy or in combination with other antidiabetic agents, including metformin or sulfonylureas, when diet and exercise alone do not achieve adequate glycemic control. The medication aids in lowering fasting and postprandial blood glucose levels, contributing to improved glycemic parameters such as HbA1c. Patients should be monitored for potential adverse effects, including weight gain, edema, and risk of heart failure, especially in those with pre-existing cardiac conditions.
Liver function tests are recommended prior to initiation and periodically thereafter due to rare reports of hepatotoxicity.
