Irban 5mg Tablet contains Ivabradine, a selective inhibitor of the If current in the sinoatrial node, used primarily to manage chronic stable angina pectoris and heart failure with reduced ejection fraction. Ivabradine acts by reducing heart rate without affecting myocardial contractility or intracardiac conduction, thereby decreasing myocardial oxygen demand and improving exercise tolerance in patients with angina. It is indicated for patients who are in sinus rhythm with a resting heart rate of at least 70 beats per minute and who are either intolerant to or inadequately controlled by beta-blockers. Additionally, Ivabradine is beneficial in heart failure patients with a heart rate above 70 bpm despite optimal standard therapy, including beta-blockers, ACE inhibitors, and diuretics. The usual dosage is 5 mg twice daily, adjustable based on heart rate response and tolerability, with a maximum dose of 7.5 mg twice daily. Common adverse effects include bradycardia, luminous phenomena (phosphenes), and headache. Ivabradine is contraindicated in patients with acute decompensated heart failure, sinoatrial block, severe hypotension, or pacemaker dependence.
