Metformin, a biguanide, primarily acts by decreasing hepatic glucose production, enhancing insulin sensitivity, and improving peripheral glucose uptake and utilization.

Teneligliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, increases incretin levels, thereby stimulating insulin secretion and suppressing glucagon release in a glucose-dependent manner. The extended-release formulation allows for once-daily dosing, promoting better patient adherence and stable plasma drug concentrations. Glytrin-Met Tablet ER is indicated as an adjunct to diet and exercise to improve glycemic control in adults inadequately controlled on metformin alone or in combination with other antidiabetic agents.
It should be used cautiously in patients with renal impairment, hepatic dysfunction, or a history of lactic acidosis. Common adverse effects may include gastrointestinal disturbances such as nausea, diarrhea, and abdominal discomfort, which often diminish with continued use. Regular monitoring of blood glucose levels, renal function, and potential side effects is recommended during therapy.
