Gluvilda M +500 Tablet SR is a combination oral antihyperglycemic agent containing Metformin 500 mg and Vildagliptin 50 mg, formulated for sustained release to enhance glycemic control in patients with type 2 diabetes mellitus. Metformin, a biguanide, primarily acts by decreasing hepatic glucose production and improving insulin sensitivity in peripheral tissues, thereby reducing fasting and postprandial blood glucose levels. Vildagliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, enhances the incretin system by preventing the degradation of glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP), resulting in increased insulin secretion and decreased glucagon release in a glucose-dependent manner. The combination of these two agents targets multiple pathophysiological defects of type 2 diabetes, providing improved glycemic control compared to monotherapy. Gluvilda M +500 Tablet SR is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes inadequately controlled on metformin alone or in combination with other antidiabetic agents. It should be used with caution in patients with renal or hepatic impairment, and regular monitoring of renal function is recommended. Common adverse effects include gastrointestinal disturbances and mild hypoglycemia.
