Metformin, a biguanide, primarily reduces hepatic glucose production and improves peripheral insulin sensitivity. Pioglitazone, a thiazolidinedione, enhances insulin sensitivity in adipose tissue, muscle, and the liver by activating peroxisome proliferator-activated receptor gamma (PPAR-γ). Sitagliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, increases incretin levels, thereby promoting glucose-dependent insulin secretion and decreasing glucagon release.
