Glimestar-PM2 Tablet ER is a combination oral antidiabetic medication containing Glimepiride 2mg, Metformin 500mg, and Pioglitazone 15mg, formulated as an extended-release tablet to provide sustained glycemic control in patients with type 2 diabetes mellitus. Glimepiride, a sulfonylurea, enhances pancreatic insulin secretion, thereby reducing blood glucose levels. Metformin, a biguanide, primarily decreases hepatic glucose production and improves peripheral insulin sensitivity, contributing to better glycemic regulation without causing significant hypoglycemia. Pioglitazone, a thiazolidinedione, acts as an insulin sensitizer by activating peroxisome proliferator-activated receptor gamma (PPAR-γ), improving glucose uptake in adipose tissue and muscle. The combination therapy targets multiple pathophysiological defects of type 2 diabetes, offering improved efficacy over monotherapy. Glimestar-PM2 is indicated for patients inadequately controlled on diet, exercise, and monotherapy with either agent. The extended-release formulation allows once-daily dosing, enhancing patient compliance. Caution is advised in patients with hepatic or renal impairment, congestive heart failure, or a history of bladder cancer due to pioglitazone’s risk profile. Common adverse effects include gastrointestinal disturbances, hypoglycemia, and weight gain.
