Glimepiride, a sulfonylurea, stimulates pancreatic beta-cell insulin secretion, thereby reducing blood glucose levels. Metformin, a biguanide, primarily decreases hepatic glucose production and enhances peripheral insulin sensitivity, contributing to improved glucose uptake and utilization. Pioglitazone, a thiazolidinedione, acts as an insulin sensitizer by activating peroxisome proliferator-activated receptor gamma (PPAR-γ), which modulates the transcription of insulin-responsive genes involved in glucose and lipid metabolism.
