Atorvastatin, a selective HMG-CoA reductase inhibitor, reduces cholesterol synthesis in the liver, leading to decreased levels of low-density lipoprotein cholesterol (LDL-C) and total cholesterol, while moderately increasing high-density lipoprotein cholesterol (HDL-C). Fenofibrate, a fibric acid derivative, primarily lowers triglyceride levels by activating peroxisome proliferator-activated receptor alpha (PPARα), enhancing lipolysis and elimination of triglyceride-rich particles. The combination therapy is particularly beneficial in patients with persistent elevated triglycerides and LDL-C despite monotherapy, reducing the risk of cardiovascular events.
