Fenofibrate works by activating peroxisome proliferator-activated receptor alpha (PPARα), which increases the oxidation of fatty acids and reduces the production of triglycerides and very low-density lipoprotein (VLDL) cholesterol. This action helps lower elevated levels of triglycerides and low-density lipoprotein (LDL) cholesterol while increasing high-density lipoprotein (HDL) cholesterol, thereby improving the overall lipid profile.

The tablet is usually taken once daily with food to enhance absorption and minimize gastrointestinal discomfort.
Common side effects may include gastrointestinal symptoms, headache, and muscle pain, while serious adverse effects are rare but can include liver enzyme abnormalities and rhabdomyolysis.
