Korandil 5 Tablet contains Nicorandil 5 mg, a vasodilatory agent used primarily in the management of angina pectoris. Nicorandil acts through dual mechanisms: it donates nitric oxide, leading to the activation of guanylate cyclase and increased cyclic GMP levels causing venous and arterial dilation, and it opens ATP-sensitive potassium channels in vascular smooth muscle, resulting in hyperpolarization and relaxation. This dual action reduces myocardial oxygen demand by decreasing preload and afterload, thereby alleviating ischemic chest pain. Korandil 5 is indicated for the prevention and long-term treatment of chronic stable angina and is particularly beneficial in patients who are refractory to conventional anti-anginal therapies. It may also be used in certain cases of unstable angina under specialist supervision. The drug is generally well tolerated; however, common adverse effects include headache, dizziness, flushing, and gastrointestinal disturbances. Rare but serious side effects such as ulceration and fistula formation have been reported, necessitating prompt medical attention if symptoms occur. Korandil 5 should be used with caution in patients with hypotension, severe hepatic or renal impairment, and those on concomitant nitrate therapy. Dosage and administration must be individualized based on clinical response and tolerability.
