Ivabradine acts by specifically inhibiting the If current in the sinoatrial node, resulting in heart rate reduction without affecting myocardial contractility or intracardiac conduction. This heart rate reduction decreases myocardial oxygen demand, thereby alleviating angina symptoms and improving exercise tolerance. Ivarest 5 is indicated for patients with stable angina who have a normal sinus rhythm and a heart rate above 70 beats per minute, particularly when beta-blockers are contraindicated or insufficient. Additionally, it is prescribed as an adjunctive therapy in patients with chronic heart failure with reduced ejection fraction (HFrEF) who remain symptomatic despite optimal standard treatment, including beta-blockers.
