It is indicated for the treatment of various systemic and superficial fungal infections caused by susceptible organisms, including dermatophytosis, onychomycosis, blastomycosis, histoplasmosis, and aspergillosis. Itraconazole exerts its antifungal effect by inhibiting the cytochrome P450-dependent enzyme lanosterol 14-α-demethylase, which is essential for the synthesis of ergosterol, a critical component of fungal cell membranes. Disruption of ergosterol synthesis leads to increased membrane permeability and ultimately fungal cell death. Italom 200 Capsule is administered orally and exhibits good bioavailability, with absorption enhanced when taken with food. The drug is extensively metabolized in the liver via the CYP3A4 pathway and has a half-life of approximately 24 to 42 hours, allowing for once or twice daily dosing depending on the indication. Caution is advised in patients with hepatic impairment or congestive heart failure, and potential drug interactions should be carefully monitored due to its influence on cytochrome P450 enzymes.
