Metformin, a biguanide, primarily reduces hepatic glucose production and improves insulin sensitivity, thereby enhancing peripheral glucose uptake. Pioglitazone, a thiazolidinedione, acts as an insulin sensitizer by activating peroxisome proliferator-activated receptor gamma (PPAR-γ), which improves insulin action in adipose tissue, muscle, and the liver. Sitagliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, increases incretin levels, leading to enhanced insulin secretion and decreased glucagon release in a glucose-dependent manner.
