Fenofibrate, a fibric acid derivative, primarily reduces triglyceride levels and increases high-density lipoprotein cholesterol (HDL-C) by activating peroxisome proliferator-activated receptor alpha (PPARα), which enhances lipolysis and elimination of triglyceride-rich particles.
